If the physical is done after May 1, it is good for the next school year. The NSAA requires schools to use their forms. The following students are REQUIRED to have physicals on file in the office of their respective building before participating in any sport:
4th, 5th, & 6th graders participating in PAL volleyball and/or basketball
All 7-12th graders participating in any sport
All 7-12th graders participating in cheer and/or dance
ALL 7th graders must have a physical on file in the MS/HS office prior to attendance regardless of participation in activities
The NSAA Pre-Participation Physical Form MUST BE USED. -

Please return your application to your building secretary. All applications will be approved/denied in the order they are received and you will receive a letter in the mail letting you know if your child(ren) qualify.

The annual health update needs to be completed in March at re-enrollment and in August at the beginning of the school year.

Parents of all students grades 7-12 participating in any of the following activities MUST complete the NSAA Student & Parent Consent Form prior to participation in those activities.
Football, Volleyball, Cross Country, Girls Golf, Softball, Play Production(One-Act Play), Music(Instrumental and/or Vocal)
Basketball, Wrestling, Swimming, Speech
Track, Boys Golf, Baseball, Tennis, Soccer -
Safe Environment Training
All people who have any contact with children in a Catholic school in the Archdiocese of Omaha must receive the Archdiocesan Safe Environment Training and be approved by the Archdiocese of Omaha prior to having contact with the children in the schools.
